This booklet will give you and the person you are looking after information, guidance and suggestions to help you both to prepare for admission to hospital and eventual discharge. The guidance covers returning home after a hospital stay, as well as discharge to another care setting.

The purpose of the booklet is to empower you to ask questions and make sure you receive the information you need. The booklet is not intended as a strict checklist requiring every item to be completed. Instead, you are encouraged to read through the guide and use it as a prompt, to consider issues that perhaps you had not thought about.

This booklet has been co-produced with Carers, Carers Voice and practitioners working across health and social care in Norfolk and Waveney.
